  • Personal & Companion Care

    OSH personal care and companion services are designed so our caregivers can go the extra mile to help you life live on your own terms.

    Companion Services

    Our companion care services are designed to match you or your loved one with someone compatible, compassionate to your needs and shares similar interests. Companions help with, but not limited to, chores, light housekeeping, providing company, as well as assisting in transportation to and from appointments but companions are not able to assist with any hands on personal care assistance.

    Personal Care

    Open System’s personal care services are designed to give assist with all tasks that have become difficult to perform on your own. These services include:

    • Bathing assistance 
    • Grooming and hygiene 
    • Dressing assistance
    • Getting in and out of bed
    • Home safety
    • Medication reminders
    • Meal preparation
    • Appointment escort
    • Light housekeeping 
    • Laundry  
    • Transportation to and from medical appointment, grocery store and other errand running.
  • Skilled Nursing Services 

    Our home care services provide you with essential care to help you stay close to your loved ones in the comfort of your own home. For those who need skilled nursing at home delivered by a License Practical or Registered Nurse, our skilled nursing services offer the flexibility of home medical care, when you need it. Depending on your specific needs, flexible options for our skilled nursing services include:

    Private Duty. For extended hourly and long term nursing needs, we’ll help partner you with a competent compassionate private duty nurse for 4-24 hours each day.

     

    Intermittent Services.For those in need of home health nursing visits, intermittent services may be the right fit for you. These visits can be very short or last up to 2 hours.

     

    These services include but are not limited to the following:

    • Intravenous infusions
    • G-tube Feeds
    • Tracheotomy Care
    • Medication management
    • Patient assessments
    • Teaching visits
    • Diabetes management teaching and care
    • Assistance in coordinating home medical equipment, pharmacy and supplies
    • Wound care/wound VAC
    • Ventilator Care
    • Monitoring vitals
  • Pediatric Care

    Our pediatric home care services allow your child to receive the care they need in the comfort of their home and in their community. Based off each individual’s specific needs, we match you or your loved one with a skilled nurse, personal care aide, or therapist ranging for a short visit or up to extended hourly care lasting up to 24 hours each day.

    Intermittent Services: For those in need of home health nursing visits, intermittent services may be the right fit for you.

  • Specialized Therapy

    Often therapies can be provided at home to recovery from injury or surgery.  We focus on providing physical therapy, occupational therapy and speech therapy. Home based therapy allows individuals to develop independence with daily activities in the comfort of their homes and avoid the hassle of outpatient settings. 

    Physical Therapy

    Remediates impairments and promotes mobility, function, and quality of life through examination, diagnosis, and physical intervention.

    Occupational Therapy

    The use of assessment and treatment to develop, recover, or maintain the daily living and work skills of people with a physical, mental, or cognitive disorder.

    Speech Therapy

    The evaluation and treatment of communication disorders and swallowing disorders.

Purpose/Procedure:

1. On admission the Agency Representative provides the patient/family a written copy and orally reviews the Agency Non-Discrimination Policy.
2. The client is notified in writing of the person they are to notify at the Agency if a problem or concern occurs.
3. All patients and staff are informed upon admission/orientation that complaints of discrimination may be filed with the Office of Equal Opportunity, Pennsylvania Department of Health, and/or the Pennsylvania Human Relations Commission.
4. The client’s signature on the Consent form and Non-Discrimination Policy indicate the receipt and review of this information.
5. Reasonable accommodations will be made for disabled employees.
